Ultherapy PRIME: Questions and Answers
Answers about Ultherapy PRIME, the newer generation of the Ulthera system: what changed, what did not, and how to decide between PRIME and the original Ulthera. Violacell Plastic Surgery offers both, and Dr. Kim plans either one from the image and from palpation, layer by layer.
What Ultherapy PRIME Is
What is Ultherapy PRIME?
Ultherapy PRIME is the newer generation of the Ulthera system for microfocused ultrasound with visualization. It works on the same principle as the original: focused ultrasound heats small points at fixed depths below the skin to tighten tissue and stimulate new collagen, while an ultrasound image shows the layers being treated. The main changes are in the imaging and the workflow, with a clearer and deeper image on a larger screen. Violacell Plastic Surgery offers both PRIME and the original Ulthera.
Is Ultherapy PRIME a new device or an upgrade of Ulthera?
It is more accurately understood as a new generation of the same platform rather than a different kind of treatment. The energy, the idea of focused heated points and the depth-based transducers carry over from the original Ulthera. What changed is the system around them: the image quality, the imaging depth, the screen and the speed of the workflow. For a patient, the treatment feels familiar, while the doctor has a clearer view of the tissue.
Who makes Ultherapy PRIME?
Ultherapy PRIME is made by Merz Aesthetics, the company that also makes the original Ulthera system. Its Asia launch was held at the Grand Hyatt Seoul in 2025 and was opened by Merz Aesthetics Asia Pacific. Because the same manufacturer produces both generations, the treatment principle and the transducer depths are consistent between them. Questions about the specific system used for your treatment can be asked at consultation.
Is Ultherapy PRIME the second-generation Ulthera?
Yes, in Korea Ultherapy PRIME is commonly described as the second-generation Ulthera, and Dr. Kim's own column on the subject uses that framing. The term simply marks it as the successor to the original system. It does not mean the treatment is twice as strong or that the original is obsolete for every patient. What matters is how either system is used on your face.
Does Ultherapy PRIME work the same way as Ulthera?
Yes. Both deliver microfocused ultrasound to a fixed depth set by the transducer, creating rows of small heated points that contract collagen and trigger remodeling over the following months. Both show an ultrasound image before each line so the doctor can see the tissue underneath. Dr. Kim notes that the time each pulse takes to deliver energy has not changed with PRIME. The differences lie mainly in how well and how deeply the tissue can be seen.
Is Ultherapy PRIME FDA-cleared?
Ultherapy PRIME belongs to the Ulthera platform, whose FDA clearances cover lifting the eyebrow, lifting loose tissue under the chin and on the neck, and improving lines on the décolleté. Regulatory approvals are granted per device and per country, so if this matters to you, ask the clinic which approvals apply to the system used for your treatment. An approval describes what the device was tested for; it does not promise any individual result.
Was Dr. Kim at the Ultherapy PRIME launch?
Yes. Dr. Kim was invited to the Asia launch of Ultherapy PRIME, held at the Grand Hyatt Seoul in 2025, and he writes that invitations for doctors in Korea were limited to 50. He has written about the event on his blog and in his columns for Violacell. An invitation reflects the manufacturer's choice of guests, so it is a fact about his involvement with the device rather than a measure of anyone's results.
What did Dr. Kim take from the Ultherapy PRIME launch?
Writing after the launch, Dr. Kim described it as an event that pointed to the direction aesthetic treatment should take. In his words, lifting has entered a period in which results depend on how precisely each person's skin structure is understood and how well the design is tailored to it. That view matches his layer-by-layer approach: the device supplies a better image, but the plan still has to be built for the individual face. His comments are summarized on the Ulthera page.
What areas can Ultherapy PRIME treat?
PRIME is used on the same areas as the original Ulthera: the brow and forehead, temples, cheeks, the lower face and jawline, under the chin, the neck and the décolleté. Around the eyes, treatment stays outside the bony rim of the eye socket. Each area has its own tissue thickness and nerve paths, so depths and line numbers are planned by zone. The clearer image helps confirm what lies under each placement, but the anatomical limits of each area stay the same.
Can Ultherapy PRIME be used on the neck and under the chin?
Yes. The neck and the area under the chin are part of the Ulthera platform's cleared uses, and PRIME treats them with the same depths. In the neck, the deeper depth is used where the platysma muscle lies at that depth and the middle depth for the skin, staying away from the midline structures of the throat. A clearer image helps judge tissue thickness here, where it varies more than people expect. Strong vertical bands and hanging skin still tend to need surgery.
PRIME Compared With the Original Ulthera
What is the difference between Ulthera and Ultherapy PRIME?
The treatment principle is the same; the main difference is the image. Dr. Kim writes that the key change in PRIME is ultrasound resolution, because older imaging showed the deepest layers less clearly. As described by the clinic, PRIME displays tissue to about 8 mm compared with about 4.5 mm on the original, and its workflow is faster. A side-by-side summary is in Ulthera vs Ultherapy PRIME.
Is Ultherapy PRIME stronger than Ulthera?
Not in the sense of delivering a more powerful kind of energy. Both systems heat tissue in the same way, with transducers set to the same depths, and Dr. Kim notes that the time each pulse takes to deliver energy is unchanged. What PRIME improves is the ability to see where the energy should go. A treatment can feel or look stronger when it is better placed, but that comes from the plan rather than from extra power in the device.
Are results better with Ultherapy PRIME than with Ulthera?
Not automatically. The collagen response after heating is the same biology on either system, so a poorly planned PRIME session is unlikely to outperform a well planned Ulthera session. The clearer, deeper image can help the doctor choose depths and avoid unsuitable areas more precisely, which may support a better-matched result. Age, skin quality, the degree of sagging and healing still shape the outcome on both systems.
Does Ultherapy PRIME hurt less than the original Ulthera?
Many people find it somewhat more comfortable, though it is not painless. Dr. Kim has written that, in his experience, the 4.5 mm depth is less uncomfortable on PRIME than it was before, and the faster workflow can shorten the total time spent in discomfort. Sensitivity still varies between people and between areas, and bony zones remain the most uncomfortable. Pain control is planned in the same way for both systems.
Is Ultherapy PRIME faster than Ulthera?
The overall treatment tends to feel faster, but not because each pulse is quicker. Dr. Kim notes that the actual time for energy delivery has not changed; what has improved is the workflow around it, including a larger screen and a quicker touch response. The clinic describes PRIME as faster in use, which can shorten the time spent in discomfort. Total treatment time still depends mainly on how many areas and lines are planned.
Does Ultherapy PRIME use the same depths as Ulthera?
Yes. As described by the clinic, PRIME uses transducers at 1.5 mm for the upper dermis, 3.0 mm for the deeper dermis and 4.5 mm for the supporting SMAS layer, the same three depths as the original. What reaches further, to about 8 mm, is the image. Dr. Kim combines the three depths according to skin thickness and structure on either system, as explained on the design and depths page.
Is Ultherapy PRIME quieter than the original Ulthera?
Dr. Kim has written that PRIME runs with less noise than the earlier system, along with a larger screen and faster touch response. For some patients, a quieter device makes the session feel less tense, especially when they are anxious about the sensation of each line. The sound is a minor factor compared with comfort, depth and planning. Noise and workflow do not change how the tissue responds.
Does Ultherapy PRIME last longer than Ulthera?
There is no good reason to expect a longer-lasting effect from the device alone. The heated points heal and remodel in the same way on both systems, so how long the change lasts depends more on your age, tissue quality, the amount of sagging, weight changes and the plan. In his answers, Dr. Kim describes the effect of Ulthera as typically holding for around six months, and he sets expectations for PRIME on the same basis. Ongoing aging continues on either system.
Can Ultherapy PRIME treat deeper than 4.5 mm?
The deepest treatment depth described by the clinic for PRIME is 4.5 mm, aimed at the SMAS. The extended reach to about 8 mm refers to how far the image can show, which lets the doctor see the deep fat, muscle and bone beneath the treatment depth. Seeing that deeper anatomy helps decide whether the 4.5 mm depth is appropriate in each spot. It does not mean energy is placed into the deepest layers on the screen.
Is the original Ulthera being discontinued?
Dr. Kim has written that the manufacturer no longer sells the original Ulthera system, so it is gradually becoming less common, while PRIME is so far installed in some clinics. Existing original systems remain in use, and Violacell offers both. For patients, this mostly means that either option may be available today, and the decision can be based on the face and the plan rather than on which system is newer.
Why does Ultherapy PRIME tend to cost more?
Dr. Kim has noted that the PRIME device and its transducers are more expensive than the earlier system, so the treatment may feel more costly to patients. Fees depend on the system, the areas and the size of the treatment, and current details should be confirmed directly with the clinic. A higher cost does not by itself mean a larger effect. The deciding factor is whether the deeper image adds something useful for your particular face.
Imaging and Planning With PRIME
Why does a clearer image matter for Ulthera treatment?
Because the right depth in one spot can be wrong in the next. Dr. Kim writes that the depth of the SMAS, the position of fat and the shape of the deep cheek fat differ from person to person, and that the earlier image showed the deepest layers less clearly. A sharper view of these layers helps confirm whether the target is really present at the transducer's depth. It also makes it easier to keep energy off bone and away from areas where tissue is too thin.
What can Ultherapy PRIME show on screen up to about 8 mm?
Below the treatment depths, the deeper image can show the full thickness of the fat, the deep cheek fat, muscle such as the masseter, and the surface of bone. Seeing these together gives a better sense of how the face is built beneath each placement. For example, it can show whether a bulge along the jawline comes from fat, from the salivary gland or from muscle. That understanding guides which depth to use and where to hold back.
How does PRIME help with the deep cheek fat?
The deep cheek fat sits beneath the supporting layer and changes in shape and volume from person to person. Dr. Kim has named it as one of the structures the earlier image showed less clearly and that PRIME displays better. Knowing where it lies helps decide whether deep energy in the cheek is wise, particularly in faces where loss of fullness would be unwelcome. In leaner faces, the plan can be kept lighter in this region.
Does a better image make Ulthera safer?
It can help. A clearer image makes it easier to confirm tissue thickness, avoid placing energy on bone and check that the transducer is in even contact, all of which reduce the chance of unwanted effects. However, ultrasound does not reliably show nerves or small vessels, so surgical knowledge of where nerves run is still needed. Side effects such as swelling, tenderness and, rarely, nerve irritation can still occur on either system.
Does Dr. Kim still use palpation with Ultherapy PRIME?
Yes. The image, however clear, shows a thin slice under the transducer at one moment, while palpation shows how thickness, fat and bone contours change across the whole face and how the tissue moves. Dr. Kim designs treatment from both on either system. As the Ulthera page puts it, a deeper or clearer image helps the plan but does not replace judgement. More on his approach is on why Dr. Kim for Ulthera.
Does Ultherapy PRIME change the shot plan?
The planning principle stays the same: lines are distributed by area and depth to fit the anatomy, and the total follows from the diagnosis. What PRIME can change is the confidence of individual choices, such as whether a zone has enough tissue for the deep depth or where fat sits in a fuller lower face. The result may be a slightly different balance of depths in some areas. The number of lines is still set for your face, not for the device.
Does a clearer PRIME image help in a thin face?
It can. In a thin face the main questions are how much tissue lies over bone and whether deep energy risks making a lean area look hollower. A clearer view of the layers helps answer those questions spot by spot, so deep lines can be kept to areas where the tissue really allows them. The cautious approach to lean faces is the same on both systems; the image simply supports it.
Does Ultherapy PRIME help with a fuller lower face?
It can help decide what kind of treatment the fullness needs. Dr. Kim has written that people who want a slimmer jawline may differ: one needs the SMAS lifted, another needs the fat layer reduced, and the salivary glands and chewing muscle also change the design. Seeing these structures more clearly on the deeper image helps tell them apart. If the fullness comes mainly from muscle, energy alone is unlikely to slim it.
Can the PRIME image show my previous fillers or threads?
It may show changed tissue or some injected materials under the transducer, but it is not a reliable way to map every product. Some fillers and threads are hard to see on ultrasound, and small amounts can be missed. You should still tell the doctor what you have had, where and when, so energy can be kept away from recently treated areas. The examination by hand also helps locate firmer tissue left by earlier treatments.
Will I notice the difference between PRIME and Ulthera on the screen?
If you have had the original Ulthera before, you may notice that the PRIME screen is larger and the deeper layers look sharper. Dr. Kim shows patients the image during treatment and explains what each layer is, so you can see where the fat sits and why a particular area bulges. Most patients cannot judge image quality themselves, but the explanation helps them understand the plan. The image is a working tool for the doctor rather than a measure of the result.
The Treatment Day With PRIME
How long does an Ultherapy PRIME treatment take?
As described by the clinic, a typical Ulthera or Ultherapy PRIME treatment takes about 20 to 40 minutes, depending on the areas treated. Time for numbing cream, photos and consultation comes on top of that. The faster PRIME workflow can trim the treatment itself, but the number of areas and lines remains the main factor. The clinic confirms timing when you book.
What anesthesia is used for Ultherapy PRIME?
As described by the clinic, numbing cream is the standard comfort measure for both Ulthera and Ultherapy PRIME, and other options can be discussed at consultation. The choice depends on your health, your medicines, the areas treated and whether you need to travel alone afterwards. Lowering energy too far to reduce discomfort may reduce the effect, so comfort is balanced against the plan. Details are on the pain and recovery page.
Is recovery after Ultherapy PRIME different from Ulthera?
Recovery is generally similar, because the tissue is heated in the same way. Redness, mild swelling and tenderness are common for hours to a few days, especially along the jaw, and bruising or tingling can last longer in some people. Most people return to normal activities the same day. Aftercare follows the same principles for both systems.
Does the faster PRIME workflow mean a shorter appointment?
Somewhat, but not dramatically. The time saved comes from the system's handling, such as the screen and touch response, while numbing, marking the plan and checking the image at each placement still take the same care. Dr. Kim notes that energy delivery itself is no quicker. Allow the same overall visit time as for the original Ulthera.
Can I switch from Ulthera to Ultherapy PRIME for my next session?
Yes. Because both systems use the same depths and principle, your response to a previous Ulthera session is useful information for planning a PRIME session. Bring details of the earlier treatment, including roughly how many lines were used, which areas were treated and how you responded. Areas that changed little or stayed tender may be planned differently next time.
Should I try PRIME if Ulthera did not work well for me?
The clinic lists people who were not satisfied with a previous Ulthera result among those who may consider PRIME. Before switching, though, it is worth finding out why the earlier treatment fell short: the main problem may have been volume loss or skin quality rather than laxity, the plan may have been too light, or the sagging may be beyond what a device can lift. Dr. Kim has written that energy devices depend on the body's own regeneration, which declines with age. A clearer image helps, but it cannot fix the wrong choice of treatment.
How soon after Ulthera can I have Ultherapy PRIME?
The same interval considerations apply as between two Ulthera sessions. Collagen remodeling continues for several months after treatment, so it usually makes sense to wait until the effect of the last session can be judged, around 3 to 6 months or longer. Treating again too soon makes it hard to tell what is working. The interval is set after reviewing how your face responded.
Choosing PRIME, and PRIME at Violacell
Should I choose Ulthera or Ultherapy PRIME?
Either can give a good result when the plan fits the face. PRIME's clearer and deeper image is most useful when the anatomy is harder to read, for example in a fuller lower face, where tissue thickness varies a lot, or after previous treatments. The original Ulthera remains a sound choice for many straightforward plans. Dr. Kim can explain whether the difference is likely to matter for you at consultation.
Who benefits most from Ultherapy PRIME's deeper image?
The deeper image matters most when the structures beneath the treatment depth affect the plan. That includes fuller lower faces where fat, salivary glands and muscle need to be told apart, faces where tissue thickness changes sharply between zones, and people who have had fillers, threads or surgery in the area. In a small area with simple, thin tissue, the added image depth may make less practical difference.
Is Ultherapy PRIME worth choosing for a small area?
It depends on the area. For a small, simple zone such as part of the brow or a limited area under the chin, the original Ulthera image is often enough to plan safely and effectively. For a small area with complex anatomy, such as the jaw angle where the salivary gland and chewing muscle sit, the clearer image may still add value. Your doctor can explain whether the system choice changes the plan.
Does Violacell Plastic Surgery offer Ultherapy PRIME?
Yes. Violacell Plastic Surgery offers both the original Ulthera system and Ultherapy PRIME. Dr. Kim plans treatment on either system with the same principle: the image and the hands decide where the energy goes, by area and by depth. The clinic is on the third floor at 138 Dosan-daero, Gangnam-gu, Seoul, about four minutes' walk from Sinsa Station exit 1. Contact details are on the clinic page.
Is Ultherapy PRIME widely available in Korea?
Not yet everywhere. Dr. Kim has written that PRIME is so far installed in some clinics, while the original system is gradually becoming less common because it is no longer sold new by the manufacturer. Availability may change as more clinics adopt it. If you are planning a trip to Seoul specifically for PRIME, confirm in advance that the clinic you choose uses it.
How can I check that the device used is genuine Ultherapy PRIME?
It is reasonable to ask which system will be used and to see the device and the transducer before treatment. Genuine treatment uses the manufacturer's own transducers, which track the number of lines delivered. A clinic that is open about the system, the transducers and the planned depths makes it easier to understand what you are receiving. If anything is unclear, ask before treatment begins.
Does the doctor matter more than the device with Ultherapy PRIME?
Both matter, but the device does not plan the treatment. Dr. Kim has written that lifting results now depend on how precisely each person's skin structure is understood and how well the design is tailored to it. A clearer image gives the doctor better information; reading it, deciding which layer to treat and knowing where to hold back depend on anatomy and judgement. That is why he plans from the image and from palpation together.
How many shots are used with Ultherapy PRIME?
Shot numbers with PRIME follow the same logic as with the original Ulthera: the total depends on the areas, the depths and the amount of fat and laxity, and it is set after examination. In his answers, Dr. Kim describes figures such as around 100 shots for a small area and around 300 for a lower face carrying more fat, while stressing that distribution matters as much as the total. The device does not change how many lines your face needs.
Is the depth ratio different with Ultherapy PRIME?
There is no fixed ratio for either system. Dr. Kim's examples of combining the 4.5, 3.0 and 1.5 mm depths, such as 6:3:1 in many women and 5:3:2 in many men, come from his column comparing PRIME with the original Ulthera, and he presents them as illustrations rather than rules. With PRIME, the clearer image can refine the balance in particular zones. The mix is still set for each face by skin thickness and structure.
What should I ask about Ultherapy PRIME at consultation?
Ask whether PRIME or the original Ulthera is recommended for your face and why, and what the deeper image is expected to add in your case. Ask which areas and depths are planned, which areas will be avoided, and what result is realistic. It is also fair to ask about comfort measures and recovery. For a broader list, see questions to ask before Ulthera.
Does Ultherapy PRIME suit people with more advanced sagging?
A better image does not change what focused ultrasound can achieve. PRIME, like the original Ulthera, firms tissue that is still roughly in place; it cannot remove excess skin or reposition tissue that has descended significantly. When sagging is advanced, Dr. Kim will explain whether surgery would serve you better, even though he does not perform surgery at Violacell. See surgery or non-surgical lifting.
Is Ultherapy PRIME suitable for younger people with early laxity?
PRIME can be used in younger people with early laxity, but treating a face with little to correct usually brings little visible change. Because energy devices work through the body's own repair, some people choose to start when early laxity appears rather than wait until sagging is advanced. Whether that is worthwhile for you depends on what the examination shows, not on age alone. Dr. Kim will tell you if treatment is not needed yet.
Is Ultherapy PRIME safe?
PRIME shares the safety profile of the Ulthera platform. Common effects are temporary redness, swelling and tenderness; less common ones include bruising, welts and line-shaped marks; and, rarely, temporary nerve irritation or weakness has been reported. Correct depth, avoiding bone and higher-risk nerve zones, and even transducer contact are the main safeguards. The full list is on the results and safety page.
